Risk Assessments & Work Permits
Prepare, score and approve risk assessments on a 5x5 matrix, reuse them as a library and gate flagged jobs behind a valid one.
The risk register in E-PMS is the single list of every structured risk assessment on the vessel and across the fleet. It is the control point behind the work-order gate: a job flagged as requiring a risk assessment cannot start or complete without a valid covering assessment. Crew prepare, senior officers or the office approve.
Three Shapes of Assessment
A library assessment is attached to a maintenance job, reused every time the job runs and reviewed by its valid-until date; office-authored ones are fleet-wide and fan out to every vessel carrying the equipment. An execution assessment is made on board for one specific work-order run, usually from the start gate. A management-of-change assessment backs a change record.
Scoring on the 5x5 Matrix
For each hazard the author describes it, scores likelihood and severity, lists the controls to be put in place and scores the residual risk after those controls. Residual bands are colour-coded from low to extreme, the assessment's score is its worst hazard rather than a sum, and a residual above the acceptable band means adding controls or escalating to the office.
Approve, Review, Re-assess
Approval stamps the assessment and, if it covers a work order, unlocks that job's start. Approved assessments carry a review date, show as review due as it approaches and stop counting as valid coverage once passed. They are read-only: to change one, re-assess into a new revision that supersedes the old one, which is kept for history and stays stamped on jobs already started.
Toolbox Talks and Offline Use
When a covering assessment exists, starting the job confirms it as a toolbox talk with the executors named. The whole module works offline on the ship, and the equipment tree narrows the register to one unit's jobs, run assessments and change records.
